Common Assessments
- Writing Prompt (ELEM.WR.WP.1-6)
(K-5)
- 1-6 Scale based on rubric (6 highest, 1 lowest)
- order for entry is as follows:

- QRI - Non-Fiction (ELEM.RE.QR.NF)

- 3 - independent
- 2 - instructional
- 1 - frustrational
- High Frequency Words (ELEM.RE.HF)
(K-3)
- 0-100 scale
- You will report the number of words read correctly from the grade level list. Choose the score from 0-100 (Kinder is 0-50) from the pull down box. If your student reads with 100% accuracy, please take them onto the next grade level list. However, note that you cannot input those scores. I wish we could, but there wasn't a simple way to build this option in PS. If you do assess a different set of words, PLEASE include those scores on the HARD COPY common assessment recording form stapled inside the Common Assessment Folder that Jenny sent out to you and INDICATE WHICH GRADE LEVEL WORDS WERE ASSESSED. FYI- 3rd grade is the last set of High Frequency Assessment Words that we have.

- Please enter the INDEPENDENT reading level for each student; we are not reporting instructional levels. Levels A-Z will be available from the pull down menu.
Note: USING THE COMMON ASSESSMENT RECORDING SHEET
This is meant to help those of you who need to get all of the information together and then INPUT. Some of you will prefer to do so directly from the assessments. If you use (as mentioned above) another grade level High Frequency Word List, please place those scores here and indicate the grade level set. You have no other place to record those scores since PS is set up only for your grade level word list. I will share that in the past, there have been a few errors that we found in the assessment data and when we went back to this sheet (completed by teachers) we found that it was an entry error.
Progress Reports

- Progress Report Codes:
- 4 Progressing beyond expectation - student performs significantly and consistently above and beyond the expectation at this point in the trimester. You've provided opportunities to see the student perform. You're "wowed"! This student will be receiving an "E" on the report card at trimester's end.
- 3 Progressing as expected - student is progressing exactly as expected based on instruction and performance up to this point in the trimester. You're not worried at all. He/she is exactly where you would expect them to be. This student will most probably be receiving an "M" on the report card at trimester's end.
- 2 Progressing toward the expectation - student is approaching the expectation at this point in the trimester, although you would prefer to see more solid growth and progression. This student may receive an "A" on the report card at trimester's end the student's level of performance is not raised through more focus and attention to work and/or school/home support. There is some concern when a student scores a 2.
- 1 Not progressing as expected - student is not demonstrating progress at this point in the trimester. The individual's performance and growth are a concern to you. This student may receive an "N" on the report card at trimester's end if support and intervention aren't provided immediately.

Comments for Progress Reports and Report Cards
- Teachers are responsible to submit comments that are accurate and appropriate.
- For all comments, teachers are responsible to have them peer-edited as a final check to ensure accuracy.
